"Question: Considering that due to the restrictions it is difficult to find sheets for filling out self-responsibility declarations, especially for elderly people or from rural areas, is it allowed to reuse the declaration by changing the date, in the case of trips for frequent medical treatments or for trips to the store near the house or to the pharmacy? Depending on the possibilities, can it be cut with pen or correction paste?
Answer: Yes, to the extent that the date for which the respective declaration is completed is evident, and the reasons for travel and the route remain unchanged."
